Our bariatric psychological assessment includes:
Two 60-minute individual therapy sessions
Computerized MBMD psychological testing
Comprehensive evaluation report and collaboration with your surgeon
Coordination with other medically necessary providers
Cost & Insurance Details: We file insurance for the two individual sessions (excluding Medicaid) A $150 administrative fee covers MBMD testing, report writing, and provider collaboration.
Note: This fee is not billable to insurance due to the bundled nature of the services.
Our registered dietitians provide individualized nutrition counseling before and after surgery, focusing on:
Pre-operative nutrition education
Post-surgical meal planning and supplementation
Long-term strategies for intuitive eating
Addressing emotional eating and body image concerns
Our goal is to help you build a healthier relationship with food while supporting your long-term wellness goals. Nutritional counseling is a vital part of successful bariatric surgery outcomes and helps patients maintain physical and emotional well-being throughout recovery.
Intake paperwork must be completed before your first appointment.
Clients without completed forms may be turned away and charged a missed appointment fee. Forms are submitted through our secure Patient Portal (access provided after scheduling).
To complete forms in-office, arrive at least 45 minutes early to your first appointment.
Payment is due at time of service. Contact our billing staff for questions about insurance coverage or your financial responsibility.
Late cancellation policy: Please notify us by 9:00 AM the business day before to avoid a $90 cancellation fee.
Due to the hour-long format, late arrivals cannot be accommodated and must reschedule.
