BARIATRIC SURGERY PATIENTS


Bariatric surgery success is maximized via a team approach which includes your surgeon, a registered dietitian, and a mental health provider, all of whom specialize in bariatrics. At Chrysalis, we are proudly affiliated with the local surgical practices and New Hanover Regional Medical Center, which is considered a Center of Excellence in bariatric surgery. Our lead psychologist and registered dietitian are Integrated Health Associate Members of the American Society for Metabolic and Bariatric Surgery; they, along with the rest of the bariatric team at Chrysalis, have ample experience providing expert care for patients pre- and post-operatively.

 What to Expect:

Chrysalis Center offers two of the pre-operative requirements that must be completed before your surgeon’s office will schedule a surgery date: Nutrition Education and a Psychological Evaluation. Please call us at 910-790-9500 to initiate your pre-operative requirements. When you call, have your insurance information ready so our staff can obtain the necessary information to verify your eligibility and benefits. Upon completion, our Practice Manager, Sarah Snyder, will call you to schedule your nutrition requirement. This is offered through two separate 90-minute nutritional seminars, which are offered on Wednesday afternoons each month. The combined total for these classes is $200, which is due at the time of service (you may elect to pay $100 each time). We ask that you arrive 10 minutes early to ensure that all participants have time to check in and so the class can begin and run on schedule. Depending on your insurance benefits and our schedules, you may be eligible for individual pre-operative nutrition education. Sarah will confirm this with you after verifying your insurance.

Paperwork is required to be completed before scheduling your psychological evaluation. Please print these forms and fill them out completely. When you arrive for your nutrition visit with paperwork filled out completely, our administrative staff will then schedule you for your psychological evaluation. If you are not able to print out the paperwork at home, you may request a copy from our office and fill it out here. Please note, you will not be scheduled for your evaluation until the required paperwork is complete.

The comprehensive psychological evaluation with one of our bariatric specialists consists of two separate 60-minute individual interviews, computerized psychological testing and interpretation, collaboration with medically-necessary providers, and report-writing.  The final report will be submitted to your surgeon. We will gladly file insurance for you, with the exception of Medicaid, for the two individual interviews. The cost for this is based on your mental health benefits through your insurance carrier. There is an additional administrative fee of $150 for the computerized testing and interpretation, collaboration with your providers, and report-writing. This fee is not billable to your insurance company solely as psychological testing because the medically-necessary components of this comprehensive service extend beyond the definition of those codes.

Please take note:

  • Payment is due at time of service. For questions regarding finances, please speak with our Practice Manager to ensure your individual summary is understood since part of your balance will be dictated by your individual insurance benefits. The surgeon’s office is not responsible for quoting your balance due to Chrysalis Center.
  • Special appointment times are reserved especially for bariatric patients. If you need to cancel or reschedule your appointment, please call our office by 9:00 am the business day before your appointment in order to avoid a late cancellation charge of $60. Because the entire hour appointment is needed for the interview, if you arrive late for your appointment, you will be unable to be seen and will have to reschedule. Cancellation of your appointment may result in a delay in rescheduling because of the volume of bariatric patients being seen.
  • Verification of eligibility and/or benefit information is not a guarantee of payment. Benefits will be determined once a claim is received and will be based upon, among other things, the member’s eligibility, any claims received during the interim period and the terms of the member’s certificate of coverage applicable on the date services were rendered. The member is responsible for any payment not made by the insurance company.
